"Please note: Our monthly General Coalition Meeting has been temporarily replaced with the Connect Form Training session."

The purpose of this open house event is to introduce community-wide service providers to the new online access point for the Continuum of Care (CoC) Coordinated Entry system called the Connect Form. The event will provide service providers with a comprehensive understanding of how to guide clients through the process, how clients are prioritized, what happens when clients are ineligible, and the service provider's role in the process. Marketing strategies will also be discussed to ensure service providers stay informed and engaged.

Target Audience:

·        Community service providers

·        Social workers

·        Case managers

·        Nonprofit organizations

·        Healthcare and housing providers

·        Other stakeholders in the Continuum of Care system

Event Goals:

  • Inform Service Providers about the new Connect Form and its integration into Coordinated Entry.

  • Clarify Providers' roles in assisting clients through the process.

  • Explain Client Prioritization and service allocation based on eligibility and need.

  • Provide Tools & Resources for effective client guidance.

  • Outline Marketing Strategies for sustained engagement with the Connect Form.
